Blood Necromancer has always had that odd mix of comfort and danger in Diablo 4. You're spending life, healing it back, building Overpower windows, and trying not to get clipped while the screen turns red. Gear matters a lot here, from Uniques to well-rolled D4 items, but the small passive choices on the skill tree can be just as important. Drain Vitality is one of those passives that looks plain at first. Then you start pushing harder content, and suddenly a little extra Fortify doesn't seem so boring.

What Drain Vitality actually does

Drain Vitality is a Necromancer passive tied to Blood skills. When you hit an enemy with a Blood skill, you get a Lucky Hit chance to gain Fortify. At one point invested, the Fortify gained is modest. With more points, it becomes more noticeable, reaching up to 7.5% of your maximum life per trigger. Fortify is the grey-ish defensive layer over your life globe. Once your Fortify is equal to or higher than your current life, you become Fortified and take reduced damage. That matters, because Blood builds often want to stay Healthy for bonuses, but they're still standing close enough to danger to get punished.

Where it sits on the tree

You won't grab Drain Vitality right at the start of a fresh Necromancer. It sits deeper in the skill tree, off to the side once you've unlocked that later section. The nice part is that getting there doesn't feel like a tax. Coalesced Blood sits nearby, and that passive boosts your damage while Healthy. Blood Necromancers already try to stay above that Healthy threshold, which means 80% life or higher, so the route makes sense. You're not wasting points just to reach Drain Vitality. You're picking up something useful on the way, which is always a good sign when planning an endgame setup.

When the passive feels worth it

Drain Vitality is at its best when your Blood skills hit often. That's why Blood Surge players tend to get more from it than most. Blood Surge can strike large packs quickly, and with Cruor's Embrace in the mix, the build throws out plenty of Blood-based hits. More hits means more Lucky Hit checks. More checks means more chances to Fortify. It's not flashy, and it won't make your damage numbers jump on the training dummy, but in Nightmare Dungeons or Pit runs you'll feel the difference. You take a bad hit, heal back up, and the Fortify layer helps keep the next one from ending the run.

When you can skip it

Not every Blood build needs Drain Vitality. Blood Wave setups, for example, don't trigger it as smoothly because the main skill has a cooldown and an animation commitment. You can still use the passive while leveling or while your gear is half-finished, but it may fall off once your defense comes from other places. If you land a Mutilator Plate, your Fortify needs can change fast. At that point, those passive points might be better spent on damage or utility. It's the same logic players use when sorting upgrades, whether they're farming bosses or buy cheap D4 items options for a build: keep what solves a real problem, and cut what no longer earns its slot.
